Hospitals do not give out information about the religious background of their patients. Therefore, either the patient of someone they know must inform the parish of who is in what hospital.

We have specially trained Communion ministers who can bring Communion to patients.

Only a priest can perform the sacrament of the anointing of the sick, which involves forgiveness of sins.

If someone is homebound and cannot get to church, the parish brings church to the homebound. 

As with hospitals, we must know about who is homebound. We can arrange for Communion or anointing.
